The conference will:

- Discuss the full implications of patients and citizens empowerment for health;

- Engage in round table debates with patients, ministers, government officials, parliamentarians and health and social care providers on the political commitments to patient empowerment – which countries are taking the lead and why?

- Provide an overview of different approaches to self management of chronic conditions – and identify the characteristics of successful programmes;

- Present evidence of the impact of self management programmes – not only on the health sector but in terms of overall social and economic impact – as well as on quality of life and well being;

- Show case social and technological innovations in empowerment for health;

- Adopt a Statement on patient empowerment and engagement.
